Reminder re: School Forms

The beginning of the school year means many forms to sign. The following forms are no longer available hard copy and can be filled out on your School Engage account

Please log on and complete as soon as possible so we have accurate records for our students.

  • Demographic Verification Form
  • Annual Authorization Form (combines the following forms: School District use of Personal Information; Consent for CBE Use of Student Information; Consent for use of Student Information by News Media and Outside Groups)
  • Citizenship Information Update Form
  • Volunteer registration Form

These forms to be filled out are not on your School Engage account and are attached below. Please print, sign, and return to homeroom teacher.

  • Digital Citizenship/Admirable Use Policy  (for use if your child may use digital resources/tools online while at school, in line with the recent Ministerial Order)
  • Student Health Plan  (to be completed only if a physical or medical condition may affect attendance or if medication is to be taken at school)

Athletic Updates

We applaud the many students who have been coming to Soccer try-outs and Cross Country team practices.  The Basketball seasons launches soon. Final decisions on team formation will occur within the next few days ahead. In order to participate on a school athletics team, important criteria must be met, including:

  • Demonstrating good character and citizenship in and out of class, and within the game environment
  • Consistently attending practices and games
  • Completing all needed parent consent forms

We are excited to invite students/parents to our junior and senior basketball teams home games this season at Bishop Pinkham. We kindly ask you review and respect these rules consistent with schools across our division to ensure safety of athletes and spectators:

  • Spectators are to cheer in a positive manner
  • No unparented Bishop Pinkham students are to attend games hosted at other schools. The same rule applies to student spectators from other schools attending games at Bishop Pinkham as well. Non student athletes who are dropped off will not be given access to BP
  • Parents are expected to assume active supervision of their children who are watching, and all spectators are expected to remain in the gymnasium unless using the washroom
  • Parents attending Away games with their own child are asked to care for no more students than you have seatbelts in your vehicle. This is to prevent parents from being “responsible” for students they did not bring with them to an Away game or that they do not know. We kindly ask you respect this rule at other schools.

We appreciate your support and lets show our Panther Pride to our hardworking athletes on all our teams this season!
